The regulation of gender in international sports is a deeply complex and contentious issue, marked by a long history of evolving policies, scientific controversies, and significant human rights challenges. This paper provides a comprehensive analysis of gender verification policies in international sports, tracing their evolution from rudimentary visual inspections to sophisticated genetic and hormonal assessments. It examines the current regulatory landscape, highlighting the divergent approaches of major international federations such as the International Olympic Committee (IOC), World Athletics, and FIFA, with a particular focus on policies concerning transgender and DSD (Differences of Sexual Development) athletes. The scientific underpinnings of these policies, particularly the reliance on testosterone levels and, more recently, SRY gene testing, are critically evaluated against emerging research and ethical considerations. Landmark legal cases, notably those involving Caster Semenya and Dutee Chand, are analyzed for their profound impact on sports law and the assertion of athletes' human rights. The paper explores the multifaceted perspectives of stakeholders, including athletes, on fairness, inclusion, and well-being. It concludes by underscoring the persistent challenges in balancing competitive fairness with inclusivity and human dignity, advocating for future policies grounded in robust, independent science, and developed through transparent, rights-respecting processes.Published
